

When a member of our referral pathway has a child that meets the criteria to access Taylor-Made Dreams' services, they complete a referral application form to request support. When we receive the completed referral form from the supporting organisation, we follow and comply with our due diligence process. This may mean requesting further information from the child’s doctor/ community nurse/ medical professional to clarify that the child’s diagnosis meets the charity’s criteria for accepting referrals. All referral forms must be signed by the referrer/ parents/ carers, or in the event where this is not possible, the parent/ carer can provide verbal permission to the referring professional to give the charity the go ahead to contact the child’s medical team.

Once the due diligence procedure has been completed, we arrange to meet the child and their parents/ carers, ideally in person at their home, hospital or hospice or in some cases via a Zoom meeting if this is more convenient for the family. Next, we email several forms to the family before the initial introduction meeting goes ahead:
1. Bucket list wishes form
2. A Travel grant permission form
3. A photograph/video permission form
At the introduction meeting, the Taylor-Made Dreams family support worker will inform the family of the full range of services available to the family and assess what ones are required and a priority. The family support worker will discuss with the child their unique bucket list wishes, who they might want to meet, where they might like to go, what they want to own and what experiences they want to have - It is important to say that although we always state that we cannot guarantee or promise to fulfil all of the child’s wishes, we do promise to try our best to deliver as many as we can. It’s a priority to Taylor-Made Dreams to manage the expectations of the young people and families we work with and to avoid undue disappointment in what is an already very challenging situation.
